What barriers, habits, thoughts or people could make change harder?
What strengths can you rely on?
What talents, qualities or resources do you already have that can help?
Who’s on your support team?
Which friends, family or mentors can encourage, guide or hold you accountable?
What’s your next focus?
Pick one thing to work on next. What specific action will you take?
How will you track your progress?
How will you keep a simple record of what you do and how often you do it?
How will you review and adjust?
What can you learn from your results, and how will you adapt your approach if something isn’t working?
Keep this sheet somewhere you can easily find it. Come back to it regularly to check your progress, update your answers, and remind yourself why you started. Small reviews like this keep you learning and moving forward.
